Complete Form 2 (Request for Access to Record) prescribed under the PAIA Regulations and send it to the Information Officer. Prescribed fees may apply: request fee [amount per current regulations] and access fees. We respond within 30 days, extendable once by 30 days. Refusals follow the grounds in Chapter 4 of Part 3 of PAIA. You may complain to the Information Regulator or apply to court.
